How to Claim

Find the Identity Document (ID) of the deceased and the informant, together with J.T.M.B.S Membership book of the deceased.
Inform the Head Office immediately by calling:

021 391 2978 / 021 3913932 or 082 557 1301.

The person who has been chosen to handle the paperwork of the deceased must also produce his/her I.D. document on all inquiries.
The left thumb prints of the deceased and informant has to appear on the NOTICE OF DEATH DOCUMENT (BI 1663). (You may be asked to do this yourself… use any ink pad).
(A) If the person passes away at home:

The ambulance and police need to be called to verify that it is a deceased body and that no foul play is involved. After they have given the family their respective two forms the following needs to be done: If there is medical history at the clinic or hospital, that facility must be contacted to have a BI 1663 form completed. Once the doctor had completed their part of the BI 1663 form, the services of the death registrar will be needed. The family then calls J.T.M.B.S on the above mentioned contact numbers and we will despatch a death registrar to complete the documentation.

If the person passes away at Hospital, call our offices to arrange for the death registrar to complete the documentation and transportation of the deceased home.

(B) If the person died unnaturally (e.g. Accidental, suicide, car accident, murder, etc.) the body must go to the Mortuary first for an Autopsy before a burial can be performed. The police staff at the Mortuary will issue all certificates after the autopsy.

When you have all the above mentioned documentation in your possession then make 3 copies of each document. Keep the copies in a safe place for future need. *

Contact the head office or your nearest Kaffang Depot to lodge a claim. Take your J.T.M.B.S Membership book with a copy of the death certificate to your nearest Kaffang Depot. The Kaffang and money will only be paid out on producing the above mentioned documents.

Claim Hours:

Head Office: Monday –Thursday: 08h30 – 16h00 & Friday: 08h30 – 12h00.

Depot: Monday – Sunday: Fajr to 22h00.

Arrange for a Ghaasil/ Ghaasilah to wash and Kaffang the mayet and also arrange for a Sheigh, Imaam or Moulana to officiate the burial. (J.T.M.B.S assist on request for the above).
Proceed to the cemetery burial office of your choice with the original burial / removal order, pay for the grave and state the correct time of burial.
The death has to be registered with your local Home Affairs Office. Documents to take with to Home Affairs Office: The deceased I.D. and the informants I.D. and the Notice of Death Documents (BI 1663). This can be done after the janaazah.

Conditions of Acceptance

T.M.B.S means: JAMIATUT TOLIEBIEN MUSLIM BURIAL SERVICES.
T.M.B.S will accept any Muslim couple, family, widows, widowers and single persons under the age of sixty-nine (69) years old.
(a) A couple is meant by: A husband and wife.

(b) A family is meant by: A husband, wife and all unmarried children or legally adopted children.

(d) A widower is meant by: A man who had lost his wife.

(e) A single person is meant by: Either a male or female.

(f) A single male or female with a child/children forms a family.

(g) Only A, B, C, D, E, or F are allowed on one application form. NO EXCEPTIONS.

Only applicants under the age of 69 years old will be accepted

Joining fee: R35,00, PLUS (1) month contribution of R25.00

Total: R60.00 must accompany all applications. (Subject to change)

Members will only qualify for benefits six (6) months after receiving official membership contribution book, providing regular monthly contributions were upheld.
Only the persons whose names are registered with J.T.M.B.S. are entitled to benefit.
Membership of J.T.M.B.S terminates when non-payment exceeds five (5) months.
No money will be paid out after membership has been terminated.
A child who marries automatically forfeits all benefits.
Stillborn to three (3) years old receives only cost of grave and Kaffang.
J.T.M.B.S reserves the right to consider acceptance of all applications.
Late claims will NOT be paid out after THREE (3) MONTHS from date of burial.
